Today a young lady came in with a complaint of a large bump behind her fifth toe and added that her fifth toe is turning under her fourth. She has pain when she runs and works out. This has been getting progressively worse over the past six months.

I diagnosed her with a Tailor's Bunion (also called a Bunionette). See picture below: (click any picture to enlarge)

Tailor's bunion is a prominence of the fifth metatarsal bone at the base of the little toe. The deformity received its name because tailors sat cross-legged all day with the outside edge of their feet rubbing on the ground. This constant rubbing led to a painful bump at the base of the little toe.

Usually a tailor's bunion is caused by an inherited faulty mechanical structure of the foot. In these cases, changes occur in the foot's bony framework, resulting in the development of an enlargement. The fifth metatarsal bone starts to protrude outward, while the little toe moves inward. This shift creates a bump on the outside of the foot that becomes irritated whenever a shoe presses against it. Sometimes a tailor's bunion is actually a bony spur (an outgrowth of bone) on the side of the fifth metatarsal head.

In the case of this patient, the fifth metatarsal bone was angulated toward the outside.

Treatment for tailor's bunion: I informed her all about the surgical correction of Tailor's Bunion where I make a small incision On the side of the foot and access the bone. I then cut a small wedge in the bone to change the angle toward the inside of the foot. After the bone wedge is removed, I placed a tiny titanium screw in order to hold it in place to heal.

Recovery from tailor's bunion surgery: For people with a desk job I can do the surgery on a Friday and send them back to work on Monday in a surgical shoe or boot. This patient, however, has a job in which she is on her feet doing sales. I advised her that she would need to take 2-3 weeks off of work or work at a desk if possible. She chose to wait until the Fall to perform her surgery when her work schedule is lighter and she can work in the office, rather than on her feet doing sales calls.

Treatment For Heel Pain Using Plantar Fasciitis Stretching Exercises And Orthotic Shoe Inserts


Treatment for plantar fasciitis should begin with rest, icing, and over the counter medications. As mentioned above, an orthotic is a device that can be slipped into any pair of shoes and can often relieve pain and help to reverse the damage and occurrence of plantar fasciitis. They do this by adding support to the heel and helping to distribute weight during movement. In addition to orthotics, many people consider night splints for treating this condition. These devices are worn during the night while you sleep, helping to keep the plantar fascia stretched to promote healing. Physical therapy has also become a common option.

Tears of the plantar fascia are a less commonly found injury than either a heel spur or plantar fasciitis. They usually involve larger and more abrupt forces than the forces which allow for plantar fasciitis to develop. High speed activity develop these forces more often. The force needs to be applied to the ball of the foot. Sprinting places the foot in a position in which this could happen. Soft shoes that bend in the arch may contribute. Plantar fascia tears may also occur in baseball or softball players when sliding in to a base with the foot making contact with the base.

Many people with Plantar Fasciitis experience a sharp heel pain in the morning, when taking the first steps after getting out of bed. This pain comes from the tightening of the plantar fascia that occurs during sleep. Stretching and massaging the plantar fascia before standing up will help reduce heel pain for the rest of your day! Massaging the plantar fascia can be done simply by rolling a tennisball (or rolling pin) under the foot, all the way from the heel to the toes. Keep rolling the ball or pin under the foot for about 5 minutes.

The option of surgery for treatment of plantar fasciitis is prescribed only, if the pain is very severe and if natural treatments do not cure the condition. The most common type of surgical treatment is the plantar fascia release or which is medically known as, endoscopic plantar fasciotomy. This involves the release or removal of the fascia, by preparing a small incision on the inside of the heel. Further, the inflamed tissue is either released or removed. Other commonly followed treatment is the heel spur removal. Heel spur removal aims at removing the heel spur and repairing the plantar fascia region.

Splints for plantar fasciitis should be worn overnight. They hold the foot in dorsiflexion position. This prevents the tightening of the plantar fascia tissues. The gentle stretching of the splint helps the tissue come back to its natural length over time. The stretching of the plantar fascia helps reduce the stress on the heel. It may cure a person within 2-3 days. It may also take longer depending on the injury. They are padded that provides comfort to the wearer. The adjustable night splint is helpful in gently reducing and increasing the stretch on the foot. This helps make the foot used to the night splint for plantar fasciitis.plantar fasciitis

Some people with normal arches or high arches get heel pain and plantar fasciitis as well. This is usually caused by muscle imbalances in the foot or lower leg. Some people have normal arches while standing but when they stand their arches fall. Either way, the first solution is to get custom orthotic inserts made for your shoes. This doesn't mean the cheap ones you get at a pharmacy. You need to find someone who makes custom orthotic inserts that can be made specific for your feet. These can cost from $75 to $300. This is expensive but can be very effective.

Stretching the calf muscles is an important part to reducing plantar fascia tension. The calf muscles, including the gastroc and soleus, attach to the back of the heel. Tension from these muscles pulls back on the heel, causing tension on the plantar fascia. A great way to stretch the calf the first thing in the morning, before you take your first steps, is to hook a towel on the ball of your foot and while keeping the knee straight pull back on the towel. Hold the stretch for a minimum of 30 seconds and repeat a few times.

What is the value of this stretch? The plantar fascia runs the length of the foot from the heel bone (calcaneus) to the toes. During a running stride, the plantar fascia undergoes a rather sudden lengthening and then shortening during the landing phase - much like a rubber band that is suddenly stretched and then allowed to shorten. This 'elastic' event requires the plantar fascia to be sufficiently supple and strong to handle such stress without breaking down. Insufficient elasticity in the plantar fascia combined with the tendency to over-pronate (which puts extra stretch on the plantar fascia) is a nearly foolproof formula for pf problems.
